Une dent dévitalisée traitée peut pour certaines raisons s'infectées ou faire mal. Il peut etre alors nécessaire de reprendre le traitement de la racine ou traitement endodontique. Cela peut nécessiter de devoir déposer les éléments prothétiques et de les refaire.
Pourquoi reprendre un traitement de racine ?
- Un traitement de racine qui ne va pas jusqu'au bout de celle-ci.
- Une infection au bout de la racine malgré que la dent soit dévitalisée.
- Une douleur chronique à la pression
- Une couronne est à changer et le traitement endodontique de celle-ci ne parait pas satisfaisant et même si la dent ne fait pas mal.
Quelle sont les étapes ?
En fonction des situations cliniques le retraitement peut être plus ou moins long et compliqué.
Il peut nécessiter plusieurs séances.
- Dépose du travail existant : couronnes, tenon, .... Il existe malheureusement toujours un risque dans cette phase de fracturation de la racine
- Désobturation du précédent traitement de racine : Il est parfois nécessaire d'utiliser des solvants ou des instruments mécaniques (limes).
- Nouvelle obturation : une fois bien nettoyé, le canal à l'intérieur de la racine est obturée à l'aide d'une pate étanche : la gutta percha.
- Nouvelle reconstruction prothétique : il est indispensable de refaire une reconstruction étanche (inlay, couronnes ou bridge). Une étape de cicatrisation sous provisoire peut être nécessaire pour tester ce nouveau traitement.

- Le traitement radiculaire constitue la fondation de la future reconstruction prothétique. Il doit parfois être repris car la racine est infectée.
- Refaire un traitement endodontique est un traitement minutieux parfois long et complexe. Il faut déposer et refaire ensuite l'élément prothétique. Il arrive parfois que sa reprise ne soit pas possible. Elle devra alors être extraite.
