Un inlay-core est un dispositif prothétique qui permet de reconstruire la partie visible de la dent et qui servira de support pour la mise en place d'une couronne.
Dans le cas de dents très abîmées (carie importante, fracture) il est parfois impossible de poser une couronne sur le fragment de dent qui subsiste.
Pour consolider la dent, un inlay-core est alors réalisé. C'est une pièce métallique ancrée dans la ou les racines de la dent, et dont la partie supérieure servira de support à la future couronne.
De ce fait l'inlay-core est aussi appelé « faux-moignon ».
La décision de faire appel à un inlay-core ne sera prise qu'après un examen clinique (position de la dent, état parodontal...) et un examen radiologique.
Principales indications
L'inlay-core est indiqué chaque fois que toute autre restauration classique s'avère insuffisante, soit en termes de résistance mécanique soit en termes de surface d'appui.
- Renforcement ou reconstitution d'une dent à couronner
- Traitement d'une dent servant de pilier à un bridge (nécessité d'un ancrage fort)
- Mauvaise occlusion (contact entre les deux mâchoires) et pression mal répartie
- Malpositions dentaires
La pose d'un inlay-core nécessite la dévitalisation préalable de la dent.
Un traitement approprié élimine la pulpe dentaire restante et prépare les canaux à recevoir le pivot de l'inlay-core.
Une fois l'inlay-core posé, une empreinte de l'ensemble est prise permettant la confection sur mesure de la couronne par le prothésiste. Elle est ensuite scellée comme si elle était placée sur une dent avec une reconstitution « simple ».
En plus d'une forte résistance à la pression masticatoire, l'inlay-core permet d'obtenir une bonne étanchéité au niveau de l'obturation des canaux. Par ailleurs il assure une solide jonction entre la racine et la couronne.
Comme tout matériau placé dans la bouche, l'inlay-core peut être réalisé en métal précieux. Si nécessaire il peut être fait en céramique (zirconium).
