Il existe différentes maladies parodontales et différents stades de ces maladies. Le traitement diffère évidemment selon le diagnostic établi.
Schématiquement, on peut définir 3 axes de traitement :
1/ pour les cas les moins évolués, les simples soins locaux prescris peuvent suffire
2/ pour les cas plus avancés, l'usage de la lithotritie peut être nécessaire. Cette technique consiste sous aide optiques (loupes, microscope) et trans-illumination par lumière LED, à enlever les calculs de tartre situés au-dessus et au dessous de la gencive. Cet acte de parodontie peut être comparé à ceux que réalisent les chirurgiens lorsqu'ils éliminent un calcul d'une cavité naturelle (canaux salivaires, vessie, reins, uretères, vésicule, cholédoque). Ainsi, cette « lithotritie » a pour but de traiter les conséquences de « lithiases infectieuses » du parodonte en agissant avec les mêmes précautions que prennent les chirurgiens pour respecter les tissus qui environnent le calcul.
La lithotritie parodontale est donc un acte délicat, long, nécessitant entraînement et expertise. Lorsqu'elle est réalisée dans les conditions optimales, la lithotritie parodontale est souvent suivie de gains d'attache osseuse et gingivale spectaculaires avec le confort que cela entraîne pour le patient.
De plus, suivant les résultats de l'analyse bactérienne de votre flore buccale, nous pourrons être amenés à vous prescrire des antibiotiques afin de réduire encore la charge bactérienne et donc potentialiser les effets de la lithotritie parodontale.
3/ Dans les cas les plus sévères, ou dans les zones présentant un « déchaussement » plus important, il peut être nécessaire d'avoir recours à une chirurgie appelée « lambeau d'assainissement » qui consiste à décoller complètement la gencive afin de procéder à un curetage plus en profondeur avant de refermer celle ci par des sutures.
Néanmoins, du fait des désagréments liés à toute chirurgie, nous tendons à privilégier l'approche la moins mutilante possible en associant lithotritie et lambeaux d'assainissement uniquement pour les cas le nécessitant.
Quelle est l'évolution sans traitement ?
Sans traitement, la maladie parodontale mène à la perte inéluctable des dents. Celle ci survient parfois relativement rapidement dans le cas d'une parodontite dite "agressive"
